How to Transfer Your Care to AuDHD Psychiatry
What Documents Should I Provide?
Before your consultation, we’ll get in touch to collect a few important documents and questionnaires. This ensures that your clinician has a full understanding of your current needs and history, and can make the most of your appointment time.
You don’t need to worry about pulling everything together right now—we’ll guide you through it step by step. Here’s what we typically ask for:

Submission Checklist
A copy of your original ADHD assessment report from a qualified specialist. This should meet the standards outlined in the NICE clinical guidelines.
These may include self-report forms or feedback previously submitted to your diagnosing specialist.
We’ll send you forms to complete, covering your current symptoms, mental and physical health, medication, and any other relevant background.
A standard form of ID such as a passport or driving licence, to help us confirm your identity and protect your privacy.
If there’s anything else you’d like us to see—previous care notes, therapy summaries, or simply something you think is important—you’re welcome to share that too.
NICE Guidelines For Assessment And Diagnosis
At AuDHD Psychiatry, we strictly follow the NICE (National Institute for Health and Care Excellence) guidelines when it comes to recognising and managing ADHD. These standards ensure every diagnosis is reliable, evidence-based, and in line with best practices used across the UK.
If your previous ADHD diagnosis was made without meeting these criteria—for example, if it lacked a structured assessment process, did not involve appropriate rating scales, or did not follow clinical protocols—we may not be able to transfer your care directly.
In such cases, we’ll always discuss it with you first, and where necessary, recommend a full reassessment to ensure we can provide you with the highest standard of ongoing support.

What Happens After a Diagnosis?

Once your care is transferred to us, you’ll have access to a wide range of tailored ADHD treatments designed to support your long-term wellbeing.
We offer both medical and non-medical interventions, which may include:

What to expect from the Appointment?
You’ll meet a prescribing clinician for a 45–60 minute review (video or in-clinic). We’ll go through your history, current/previous medication, goals and any concerns to ensure we have full clinical oversight. Please have your recent blood pressure, pulse, height and weight to hand.
Fee (Transfer of Care consultation): £475.
Before your appointment we review your diagnostic report and any medical records to confirm they meet NICE standards (e.g., DSM/ICD criteria, sufficient detail and recency).
If the report is very old, was completed outside the UK, or there’s uncertainty (symptoms changed/worsened, treatment no longer effective, major life changes), we may recommend a re-assessment. We’ll let you know our decision in advance and advise the most appropriate next step.
If your previous diagnosis is accepted, we’ll confirm medication safety and suitability.|
• Stable on medication: we can continue your care under our service.
• Starting/adjusting medication: we’ll begin titration—most people need 3–4 follow-ups spaced 3–4 weeks apart to find the right dose (£165 per follow-up).
We’ll guide you through each step and fine-tune as needed.
We’ll discuss whether Shared Care with your GP is possible.
• If GP accepts: NHS prescribing takes over (standard NHS charges apply). We continue 6-monthly medication reviews for safety (£275).
• If GP declines: we can continue private prescribing. Repeat prescriptions are £45 each (medication cost payable to the pharmacy). We’ll also see you every 6 months for a medication review (£275).
Your clinician will outline a plan tailored to you—this may include ongoing prescribing, therapy, ADHD coaching, and reasonable-adjustment advice. If you remain private, repeat prescriptions are issued monthly or bi-monthly as appropriate (£45 each).
Please email us about a week before you’re due to run out so we can send the repeat-prescription form. We can also prepare documentation should you wish to revisit Shared Care with your GP in future.

If your original ADHD diagnosis doesn’t meet NICE guidelines, we may not be able to continue your care and will recommend a full reassessment instead.
If you were diagnosed in childhood and have not received regular care since, you may require a new adult assessment.
